Considerably fewer keep it off, and almost everything written about this subject addresses only the first half.

Regaining most of what was lost is not an unusual outcome, it is the common one, and understanding why is more useful than another plan for the losing part.

Why the second half is harder

Losing weight has a clear structure. There is a plan, a visible measure, momentum, and the encouragement that comes from change. Maintenance has none of that. It is doing the same thing indefinitely with no feedback and no finish line, and being pleased about nothing happening.

There is also a physical component. As you become smaller, you need less energy, partly because there is less of you and partly through other adaptations. That effect is real, it is usually smaller than the fear of it, and it means maintenance sits at a slightly lower intake than you might expect. Whether anything can nudge that number back up is its own question, taken apart in whether you can actually raise your basal metabolic rate. That falling requirement is what most people mean by a slow metabolism, and our page on fixing a slow metabolism naturally explains how much of it is arithmetic and how much is adaptation.

How many people actually keep weight off?

More than the gloomiest claim suggests, and less than any before and after photo implies. The most quoted pessimism is that everyone regains everything within five years. A 2001 meta-analysis in the American Journal of Clinical Nutrition went looking for that and pooled 29 US studies of people who had completed a structured weight loss programme, with at least two years of follow up. Five years after finishing, the average participant was still more than 3 kg lighter, a reduced weight of more than 3 per cent of starting body weight. Six of the studies reported that the groups who exercised more kept off significantly more.

Three kilograms is not the headline anyone signs up for. It is also not zero, and for many health measures a sustained few per cent matters more than a dramatic figure that does not last.

The other source everyone cites is the National Weight Control Registry, a US research database of adults who have lost at least 30 pounds and kept it off for at least a year. In one analysis of its members published in Obesity Research in 2002, 78 per cent reported eating breakfast every day of the week. Registry members volunteered because they succeeded, so the registry describes what successful maintainers do, not what makes someone succeed. Eating breakfast is not shown here to cause anything.

Source

Who it describes

What it found

What it cannot tell you

Anderson and colleagues, 2001 meta-analysis

People who completed structured programmes in 29 US studies

More than 3 kg and more than 3 per cent of body weight still off at five years, on average

How any one person will do

National Weight Control Registry, 2002 analysis

Volunteers who had already lost 30 pounds or more and kept it off a year

78 per cent ate breakfast every day

Whether those habits cause maintenance or come with it

Losing

Maintaining

Feedback

Visible, frequent

Almost none

Motivation

High at the start

Has to come from habit

Structure

A defined plan

Ordinary life

Success looks like

Change

Nothing happening

Duration

Months

Indefinitely

The test of a plan is whether you could live on it indefinitely.

The mistake almost everyone makes

Choosing an approach for how fast it works rather than for whether it could be permanent.

Any sufficiently restrictive plan produces loss. The question that decides the outcome is what happens when you stop, and every plan you cannot continue eventually gets stopped. At that point the eating pattern that produced the original weight is still there, and nothing about the plan changed it.

The useful test before starting anything: could you eat like this in five years, on a bad week, during a holiday, when someone else is cooking? If not, the plan already has an ending, and the ending is regain.

choosing an approach to eating covers choosing an approach on that basis rather than on claimed mechanism.

What actually helps

Lose it slower than you want to. Faster loss means more muscle lost and less time to build the habits that carry maintenance. Impatience here is expensive later.

Keep some form of monitoring. Weighing weekly, a monthly measurement, or simply how clothes fit. People who maintain tend to notice drift at two kilograms rather than at ten.

Stay active, particularly resistance training. Regular activity is among the strategies most consistently recommended for preventing regain, and keeping muscle changes both how you look and what you can eat.

Change the eating pattern rather than suspending it. If you plan to return to how you ate before, you already know the outcome.

Recover quickly from disruption. Everyone has bad weeks. The structure disappears, since there is no longer a plan to follow. And the vigilance fades, because nothing is changing and there is nothing to monitor. Weight drifts back slowly enough that no individual week looks like a problem.

Decide what maintenance looks like before you reach it, rather than working it out afterwards.

Keep one measurement, weekly or monthly, and write it down. Memory is unreliable and drift is gradual.

Set a number at which you act, perhaps two or three kilograms above where you finished. Acting at that point is easy. Acting ten kilograms later is another whole attempt.

Keep the activity, particularly resistance training, since it is the habit most likely to be dropped and among the most predictive of maintaining.

Expect that it will not feel like an achievement, because nothing happening never does.

Life events, which are the actual test

Almost nobody regains weight during an ordinary month. It happens during illness, a house move, a new job, a bereavement, a new baby, or any period where the normal structure disappears.

That is worth planning for rather than being surprised by. A maintenance approach that only works during calm periods is not a maintenance approach. Having a minimum version, something you can hold to during a difficult few weeks, is more useful than an optimal version you abandon entirely.

The minimum version for most people is: keep moving in some form, keep protein reasonable, and keep monitoring. Everything else can lapse temporarily without much cost.

About the regain, honestly

If you have lost and regained before, possibly several times, the standard framing tells you this was a discipline failure. That framing is both wrong and actively unhelpful.

Regain is the expected outcome of an unsustainable approach. It happens to most people who try them, which is information about the approaches rather than about the people. Someone who lost weight three times has demonstrated they can lose weight three times, which is not nothing.

What changes the outcome is choosing differently, not trying harder at the same thing. why the scale moves slowly even when you eat less covers a related frustration, and weight loss versus fat loss covers why the scale is a poor sole measure.

What maintenance actually looks like

Unremarkable, which is why nobody writes about it.

Eating roughly the same way most days, with genuine flexibility rather than rules

Activity that continues because it is a habit rather than because it is motivating

Some monitoring, frequent enough to catch drift early

Weeks that go badly, followed by returning to normal rather than starting again

Not thinking about it very much, which is the actual goal

That last point is worth sitting with. The aim is not permanent vigilance. It is arriving at a way of eating and moving that you do not have to think about, which takes time to build and is the reason slow is better.

If food or your body has become something you think about constantly, or if eating feels out of control, that is worth talking to a doctor about rather than reading another article. That is not a judgement, it is just a different problem from the one this page can help with.

Frequently asked questions

Why do most people regain the weight?

Usually because the approach that lost it was never one they could continue.
