In a study of 150 adults by <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16280423\/\" target=\"_blank\" rel=\"noopener\">Johnstone and colleagues in the American Journal of Clinical Nutrition in 2005<\/a>, fat free mass explained 63 percent of the differences in basal metabolic rate between people. Fat mass added 6 percent and age 2 percent. That leaves 26 percent unexplained, which is real, but it is not a furnace.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Fat free mass is mostly organs, muscle and bone, which is a polite way of saying body size. So the person who cannot gain weight usually has a <em>lower<\/em> resting burn than the friend who gains without trying, because there is less of them to keep warm. The fast metabolism being described is almost never a measurement anyone has taken. It is a personality trait that got assigned to an organ. Researchers have fed people a known surplus in controlled conditions and measured what happened. The constitutionally thin group did not, despite a similar nitrogen balance<\/td><\/tr>\n<\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Read the middle row again, because it is the one nobody quotes. Everyone ate the same surplus. Some of them stored roughly ten times as much fat as others. Identical instructions, wildly different outcomes, and the thing that predicted the difference was how much people moved without deciding to.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"where-does-the-extra-food-actually-go\">Where does the extra food actually go?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Into fidgeting, mostly. Non exercise activity thermogenesis is the energy spent on everything that is not sleeping, eating or deliberate exercise: shifting in a chair, pacing while on the phone, tapping a foot through a meeting, taking the stairs because waiting felt boring. In the Levine trial it absorbed two thirds of the extra energy people were given.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The awkward bit is that it is unconscious. Nobody in that study chose to fidget more. Their bodies simply spent the surplus on movement they never noticed making, which is why the reader&#8217;s own account of the problem is usually accurate: they really do eat more, and it really does not stick. They were right. The explanation just was not their resting metabolism.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is also why the standard advice feels useless without being wrong. The <a href=\"https:\/\/www.nhs.uk\/live-well\/healthy-weight\/managing-your-weight\/advice-for-underweight-adults\/\" target=\"_blank\" rel=\"noopener\">NHS suggests underweight adults add around 300 to 500 extra calories a day<\/a>, which is sound, and is roughly what the top ranking pages repeat. For someone whose movement response is mild, that is plenty. At the resistant end of the Levine range, a surplus that size can be substantially absorbed before it ever reaches the scale, which is why weeks pass with nothing to show.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><thead><tr><th>What the ranking pages say<\/th><th>What they leave out<\/th><\/tr><\/thead><tbody>\n<tr><td>Eat 300 to 500 more calories a day<\/td><td>That the response to an identical surplus varies about tenfold between people, so one number cannot fit everyone<\/td><\/tr>\n<tr><td>Eat more often<\/td><td>That appetite and gut tolerance, not metabolism, are usually what stop the extra food going in<\/td><\/tr>\n<tr><td>Lift weights<\/td><td>Why lifting matters here specifically: it gives the surplus somewhere to go other than fat<\/td><\/tr>\n<tr><td>You have a fast metabolism<\/td><td>That resting metabolism tracks body size, so a smaller person usually has a lower one, not a higher one<\/td><\/tr>\n<\/tbody><\/table><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"what-do-people-who-tried-mass-gainers-actually-report\">What do people who tried mass gainers actually report?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Since the second half of the search results for this question is mostly protein and creatine brands, it is worth reading what buyers say afterwards. We read 12 customer reviews across three vendor pages on Trustpilot on 19 September 2026. This is testimony, not evidence: nobody weighed these people in a laboratory, and a review is a story told by one person on a day they felt like typing.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>On <a href=\"https:\/\/uk.trustpilot.com\/review\/www.usn-sport.com?page=2\" target=\"_blank\" rel=\"noopener\">the USN Sport page<\/a>, which carried a 1.6 score across 417 reviews when we checked, a reviewer posting as Daria wrote in May 2024 that a mass gainer left her bloated and aching, unable to eat for a while, and that she lost weight rather than gaining it.<\/li>\n<li>On the same page, a reviewer posting as Julie Jones wrote in May 2023 that returning to a mass powder brought reflux, bloating and nausea within five days, and that stopping it settled the symptoms.<\/li>\n<li>On <a href=\"https:\/\/uk.trustpilot.com\/review\/promassgainer.co.uk\" target=\"_blank\" rel=\"noopener\">the page for a gainer seller that has since closed<\/a>, a reviewer posting as Tilly Macaulay wrote in May 2022 that the product did nothing for her appetite at all. On the same day, a reviewer posting as Max Gox reported putting weight on and was happy with it.<\/li>\n<li>On <a href=\"https:\/\/www.trustpilot.com\/review\/www.primalmuscle.com\" target=\"_blank\" rel=\"noopener\">the Primal Muscle page<\/a>, a reviewer posting as Big Kahuna wrote in January 2026 that he had spent about 5,000 dollars after being given a target to reach over six months, and that he finished the period lighter than he started, with no change in strength or size. Other reviewers on that page were positive about strength.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The accounts split roughly evenly between satisfied and not, so this is a pattern rather than a verdict. What is consistent among the unhappy ones is the obstacle they describe, and it is never metabolism. It is the gut: bloating, nausea, fullness, then eating less overall than before they started, which is a tidy way to spend money on going backwards. But the reasoning changes once you accept that appetite and unconscious movement are the constraints, rather than a mystery furnace.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Calorie density does the heavy lifting, because the real limit is stomach space and willingness, not arithmetic. The NHS points to nuts, seeds, cheese, beans, fish and eggs, and to milk based drinks taken between meals rather than with them, precisely so that the extra energy does not have to arrive as extra volume at a table where you are already full. A surplus has to be partitioned somewhere, and resistance training is the signal that sends part of it to muscle. In <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28698222\/\" target=\"_blank\" rel=\"noopener\">a systematic review of 49 randomised trials covering 1,863 participants, published by Morton and colleagues in the British Journal of Sports Medicine in 2018<\/a>, protein supplementation alongside resistance training increased fat free mass, and the review reported no further gains once total protein intake passed about 1.62 g per kg of body weight a day. That figure is what the trials observed, not a target to set for yourself. A surplus is an average across weeks, not a heroic Sunday. Three days of deliberate eating followed by four days of forgetting is a maintenance diet with extra admin attached.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"when-is-trouble-gaining-weight-a-reason-to-see-a-doctor\">When is trouble gaining weight a reason to see a doctor?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <a href=\"https:\/\/www.nhs.uk\/conditions\/unintentional-weight-loss\/\" target=\"_blank\" rel=\"noopener\">NHS advises seeing a GP if you keep losing weight without changing your diet or exercise routine<\/a>. Its list of causes includes an overactive thyroid, type 1 diabetes, coeliac disease and inflammatory bowel disease, among others that are more serious again. Unexplained weight loss is a symptom that gets investigated, not a trait to be worked around with shakes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The NHS also says to speak to a GP if you use the control of food to cope with feelings. That sentence is there for a reason, and it applies whichever direction the scale is moving.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Constitutional thinness, the phenotype in the Ling study, is real: naturally underweight, with normal hormones and normal eating, and genuine resistance to gaining. It is also a description doctors arrive at after ruling other things out, not one to award yourself after a disappointing month.
